Bring the information that matters most
You do not need to bring everything, but having the right information can make a Medicare review more helpful and efficient.
- Your current coverage cards and plan information.
- A list of your doctors and specialists.
- A list of prescriptions and preferred pharmacies.
- Questions about travel, costs, extra benefits or provider access.
- Any Annual Notice of Change or employer coverage documents that affect your situation.
Protect your information. Do not send your Medicare number or other sensitive data through ordinary email unless you are instructed to use a secure process.
Optional: organize information in PlanEnroll
If you want to prepare before your appointment, you can use Michael Johnson’s secure PlanEnroll page to organize key information.
