
What changed for 2027?
Medicare negotiated prices for 15 additional high-spending Part D drug groups as part of its second round of the Medicare Drug Price Negotiation Program. The agreed prices, called maximum fair prices, become effective January 1, 2027. They follow the first 10 negotiated drugs, whose prices began in 2026.
CMS has published negotiated prices for these drugs and estimates substantial savings to Medicare. But the negotiated price is not a universal copay or a promise that every person taking the medicine will see the same savings at the pharmacy.
The 15 drug groups with negotiated prices in 2027
Some groups include multiple brand names for the same active ingredient. The 15 groups are:
- Austedo; Austedo XR — treatment of certain involuntary movement conditions.
- Breo Ellipta — treatment of asthma and chronic obstructive pulmonary disease (COPD).
- Calquence — treatment of certain blood cancers.
- Ibrance — treatment of certain types of breast cancer.
- Janumet; Janumet XR — treatment of type 2 diabetes.
- Linzess — treatment of chronic constipation and irritable bowel syndrome with constipation.
- Ofev — treatment of certain progressive fibrosing lung diseases.
- Otezla — treatment of psoriasis and psoriatic arthritis.
- Ozempic; Rybelsus; Wegovy — semaglutide medicines used for different approved indications, including type 2 diabetes, weight management and certain cardiovascular-risk indications.
- Pomalyst — treatment of certain cancers, including multiple myeloma.
- Tradjenta — treatment of type 2 diabetes.
- Trelegy Ellipta — treatment of COPD and certain forms of asthma.
- Vraylar — treatment of certain mental health conditions.
- Xifaxan — treatment of certain gastrointestinal and liver-related conditions.
- Xtandi — treatment of prostate cancer.
Drug descriptions are general, not complete lists of FDA-approved uses. A drug's appearance on the negotiated list does not mean Medicare covers it for every condition. In particular, Medicare coverage of weight-loss drugs depends on the indication and applicable coverage rules.
Will your prescription cost less in 2027?
Possibly. The negotiated price affects the underlying price available for eligible covered dispensing, but what you pay may also depend on your Medicare Part D or Medicare Advantage prescription drug plan, formulary placement, deductible, copay or coinsurance, pharmacy network and any financial assistance you qualify for.
Even if your medication is on this list, compare your specific medications, dosages and pharmacies when reviewing plans. A plan's total annual cost can matter more than its monthly premium alone.
What to do during Medicare Annual Enrollment
Medicare Annual Enrollment runs October 15 through December 7. Before making a coverage decision for 2027, gather your current prescription list, strengths and frequency; note your preferred pharmacy; and review your current plan's Annual Notice of Change. An individual review can clarify whether your prescriptions are covered, whether restrictions apply and how the new negotiated prices might affect your total costs.
